We’re sorry but your postal code is not available in our database. Please visit the Ontario Health website to identify the Network Lead Organization in your area that is responsible for coordinating referrals into the OSP program. They will help direct you to services that best meet your needs.

Have your primary care provider sign up for you:

Ask your primary care provider to complete and submit this online referral form or submit a referral form through their Electronic Medical Record (if available).

You or your primary care provider can also download a fillable PDF in Arabic, Farsi, Spanish, and Traditional Chinese, complete, and email it to us at: bb-referral@cmha-yr.on.ca
